NHTD Brooklyn

If you are qualified for a nursing home but would prefer to remain in or return to your own home, you may be eligible for the Nursing Home Transition & Diversion Waiver program (NHTD), which is funded by Medicaid. People who would otherwise need long-term care in a facility are the target audience for waiver programs. Counseling, life skills training, a structured day program, transition assistance, assistive technology, and other supplementary services are all part of the NHTD brooklyn program.

The NHTD brooklyn was established on the principle that people of all ages, especially those in their golden years and those with disabilities, have the same rights as everyone else, including the right to receive care in the most individualized and least restrictive environment.

Purpose and Understanding

The NHTD brooklyn prepared this Programme Manual so that participants could learn more about the program’s policies and procedures. Service Coordinators, other waiver service providers, RRD Centres, RRD Specialists, Nurse Evaluators, and Quality Management Specialists are the intended readers of this guide. On request, it can also be made available to waiver applicants and participants.

The rules and processes of NHTD brooklyn are outlined in Sections 1–10 of this Program Manual. Appendices A through G can be found in Section 11, and they contain the various waiver program forms. In Section 12, you’ll find a definition of each term used in the waiver process. The maps showing the areas covered by the Regional Resource Development Centres and the Quality Management Specialists can be found in Section 13.

Goals

Lowering the number of DMHAS patients admitted to nursing homes unnecessarily;

Residents of nursing homes who suffer from mental illness are helped to return to the community. DMHAS funds Nurse Clinicians and Case Managers at the organizations listed below to coordinate efforts between community providers, nursing home personnel, and hospital discharge planners toward these ends. The Medicaid Home and Community-based Waiver for People with Serious Mental Illness works closely with the state’s Money Follows the Person Demonstration Project Research and Development Facilities and Quality Assurance Experts.

Keeping people out of the ER and from needless stays in acute care hospitals due to mental health issues.

Reintegrating former residents of nursing homes into society
Individuals with serious and chronic mental illness who are currently residing in nursing homes benefit from increased community outreach, education, and engagement efforts.

Discharge teams in nursing homes can benefit from professional consultation on available behavioral health services.
DMHAS LMHAs and individuals through diversion or transition will have access to DSS’s liaison services. Money Determines the most suitable community-based option, such as a skilled nursing facility or a bed in a state hospital, based on the Person and Mental Health Waiver staff Assessment of the individual’s Level of care needs.

Knowledge of current community resources for those with mental illness and substance abuse difficulties
Instruction in self-care and preventative measures for people with diabetes

Assistance in times of crisis and liaison with emergency services and law enforcement

Alternative therapies based on mindfulness training to lessen psychiatric symptoms and drug cravings

Counselling, referral, and information services for people struggling with substance abuse
Help with establishing connections and providing MAT support.
