  • Active New York State Licensed Practical Nurse (LPN) license
  • Current CPR or BLS certification
  • Infection Control certificate
  • Professional liability (malpractice) insurance
  • Ability to pass required criminal background checks, physical exams, and drug screenings
  • Valid I-9 documentation confirming authorization to work in the United States
  • Two professional references
