- Active New York State Licensed Practical Nurse (LPN) license
- Current CPR or BLS certification
- Infection Control certificate
- Professional liability (malpractice) insurance
- Ability to pass required criminal background checks, physical exams, and drug screenings
- Valid I-9 documentation confirming authorization to work in the United States
- Two professional references
